If they meet Chase Commercial Bank requirements, the contact us submission form is displayed, otherwise they are prompted with a link to the most appropriate Chase line of business for their needs.

Designed online questionairre for corporate executive survey. Over 5 pages in length, it contained over 50 questions utilizing a full array of form objects (check boxes, radio buttons, list/menu pull-downs, etc.) and was distributed exclusively to high-level executives.
